Setting: A violent world in itself, Tokyo is now a city consumed by crime — beatings, kidnappings and murder.
Plot Summary: The story starts with a news broadcast outlining the increasing surge of criminal activity in Tokyo. Amidst all this, a beautiful lady gets in touch with a detective from the City Hunter group to locate her missing sister, Karumi. She becomes frustrated with the action, or rather the lack of it, from the police, and so she chooses to hire Ryo Saeba (Akira Koizumi), a well-known private detective and a former Special Forces soldier who, besides being a lethal and tactically brilliant soldier, has a rather peculiar flaw - he gets easily infatuated by beautiful ladies.
Ryo and Hideyuki, through their swift actions, manage to locate Karumi, the latter being in a predicament with the Yakuza. The Yakuza get down to fighting, and Ryo, with his fighting skills, manages to overcome them but Karumi gets the wrong idea and thinks Ryo is a sexual predator, thus she flees. Makimura gets a hold of some strange liquid that Karumi has dropped, and he keeps it a secret from Ryo in order to study it.
They meet Seiko Nogami during their investigation, a faithful policewoman and one of Makimura’s ex-colleagues. She tries to dissuade City Hunter from probing into Karumi’s case. Without even listening to the warning, the duo is close behind in their search, and it turns out to be their last fight where Makimura gets hit by a truck (controlled by a man that was somehow "induced" by the serum) and dies. With his last breath, he gives Ryo the makeup serum and asks him to take care of his sister Kaori.
It comes to Kaori that a month later, she requests Ryo’s assistance in examining her brother’s death. Initially, Ryo is hesitant, but when he locates the serum as having links with an old drug called angel dust, he is drawn in again. Angel dust is the drug that can turn an ordinary person into a killing machine without a brain. After 20 years, Seiko and Ryo decided this drug had to do with their discovery of the Union Syndicate, whose leader is Kunio Kono.
Kaori is absolutely determined in solving this mystery and works as a spy against Ryo at a cosplay event where the Syndicate identifies Karumi again. Despite Ryo's efforts, Karumi is kidnapped. When they track down the Union Syndicate's base, Ryo and Kaori realize the Union Syndicate's plan is to make zombies out of the casually drugged soldiers.
In the ultimate encounter in Tokyo, Ryo and Kaori storm the Syndicate’s facility. Then, they met a man called Kono, who reveals that Kaori’s biological mother was a victim of the angel dust experimental project killed by Makimura’s father. The bombshell revelation has a significant impact on Kaori and she gathers strength to confront Kono. However, the Union Syndicate members murder Kono to keep their dark secrets hidden.
The story wraps up with the destruction of the Union Syndicate and the restoration of peace. Kaori becomes Ryo’s new partner and uses the threat of getting rid of his porn collection to convince him to accept her. Karumi is back to being a cosplayer, and Ryo is still a detective.
