DEA Special Agent John “Breacher” Wharton (Arnold Schwarzenegger) is shown in a video recording in which his wife is tortured and killed. The story actually starts here, from the moment this shocking event so deeply affects Wharton’s inner world that it inspires his behavior during the entire flight.
Wharton and his DEA Special Operations Team raid a cartel house eight months after the trauma. The seized cash from the stash house amounts to $10 million. They destroyed the rest of the money and staged the aftermath of the sewer robbery by concealing the stolen amount in the sewer. However, when they returned to retrieve their money, it was gone. Since Morales' boss, Floyd Demel (Martin Donovan), is the one who knows the situation, an inquiry results in the team being put on suspension for six months. The team members thought the money was stolen by them, but no evidence of the theft is discovered, and they are later allowed to work again.

Caroline Brentwood together with her partner to Darius Jackson start investigating these mysterious killings, and they reveal a complicated situation inside the team which involves lies, deceit, and betrayal.
The clues hint that the murders have been executed by the Guatemalan Special Forces, a unit that is not only trained to be highly efficient but is also notorious for its brutality. Nevertheless, the actual perpetrator comes from the near vicinity. Lizzy, who is the antagonist in the tale, is the one behind these murders, driven by greed and betrayal, she literally gets everything she wants. In the climactic moment, Wharton faces Lizzy, who admits to stealing the money and eliminating the team members.
At the very last scene, Wharton is in Mexico chasing the murderer of his family. After a violent confrontation, he ends up shooting the guy, and then he just sits down talking with him in silence, thinking of the consequences of his revenge. The film’s final moment is of Wharton having a drink with his feet on the bar, he is a man who has finally avenged his family’s killing, but it has been his life that he lost in the process.
